How to Make a Referral to Integrity Transitional Hospital
Although referrals to Integrity Transitional Hospital come from a variety of sources, such as physicians, short-term acute care hospitals, sub-acute/skilled nursing facilities, rehabilitation facilities, managed care providers, case management companies or by the patient’s family. All referrals require a physician’s assessment and order for the patient’s medical information to be reviewed. All patient referrals to ITH are confidentially reviewed and evaluated by a licensed Nurse Liaison.
At Integrity Transitional Hospital, all patients are admitted, assigned a room and cared for without regard to race, color, creed, age, handicap, national origin or payment classification.
Referral and Admission Process
Below is the process in which a patient referral is made:
- Physician/Case Manager discuss with patient/family discharge plan to Integrity Transitional Hospital

Physician/Case Manager request patient evaluation by licensed ITH Nurse Liaison

Physician writes order for ITH to evaluate and assess clinical appropriateness

On-Site assessment performed expeditiously by ITH Nurse Liaison

Evaluation is reviewed by accepting physician

Referring Case Manager informed of decision to admit

Patient/Family meeting with ITH Nurse Liaison

Admission arrangements coordinated with patient and patient’s family, referring hospital and accepting physician
Financial Matters
Integrity Transitional Hospital is a Medicare and Medicaid approved provider and accepts most commercial insurance coverage, including the U.S military insurance carrier, TriCare. ITH has contracted with most major commercial insurance and managed care providers, regionally and nationally. ITH also accepts Worker’s Compensation benefits. What You Will Need Before You Are Admitted
When you arrive at ITH, please bring the following with you:
Government approved identification card(s)
Medicare or Medicaid cards
Any insurance cards, information or policies
Living will, power of attorney or durable power of attorney
Certificate of Guardianship, if one exists
Copy of an organ donation request card
What to Expect During Your Stay
In an attempt to make your stay at Integrity Transitional Hospital a comfortable experience, we recommend you bring with you only a few personal items, such as toothbrush and toothpaste, shampoo, pajamas or robe, and slippers. If you wear glasses, contact lenses, dentures or have a prosthetic device, they must be labeled appropriately and stored in a container. We appreciate your understanding and compliance with the fact that electric appliances are prohibited in the hospital unless pre-approval is obtained by administration.
Integrity Transitional Hospital is not responsible for valuables or money kept in your room therefore, as a precaution, we recommend you leave valuables at home.
